Steps to Download Philhealth Form Pmrf Excel
Go to the Philhealth website: https://www.philhealth.gov.ph/downloads/.
Under the Membership section, click on PMRF: Philhealth Member Registration Form.
A new tab will open with the PDF version of the form. To download the Excel version, click on the link at the bottom of the page that says "Click here to download PMRF in Excel Format".
Save the file on your computer or device. You can rename it if you want.
Steps to Fill Out Philhealth Form Pmrf Excel
Open the Excel file that you downloaded. You will see two worksheets: PMRF and Instructions. Read the instructions carefully before filling out the form.
On the PMRF worksheet, enter your personal details, such as your name, date of birth, citizenship, sex, civil status, address, contact number, and email address. All fields are mandatory unless indicated as optional.
If you are registering as a new member, leave the Philhealth Identification Number (PIN) blank. If you are updating your existing membership details, enter your PIN.
If you have a Philsys ID Number (optional), enter it as well.
If you are a foreign national, enter your passport number and country of origin.
On the next section, declare your dependents, if any. You can use additional forms if necessary. Dependents include your legal spouse, children below 21 years old, parents 60 years old and above, and children with permanent disability.
Enter their names, dates of birth, sex, relationship to you, and PIN (if any).
If you are updating or amending your membership details or dependents, check the appropriate boxes on the next section and indicate the changes from and to.
On the last section, sign and date the form. You can print it out and sign it manually or use a digital signature.
How to Submit Philhealth Form Pmrf Excel
You can submit your completed Philhealth Form Pmrf Excel in person or online.
To submit it in person, bring a printed copy of the form along with your valid ID and other supporting documents (if any) to the nearest Philhealth office or accredited collecting agent.
